Not all tightening treatments are created equal - and confusing them can cost you volume.
Here’s the nuance 👇

XERF = slow, controlled monopolar RF heating
Ultherapy HIFU = fast, targeted deep heat (with a trade-off)

Fast heat devices like HIFU can reduce facial fat over time—that’s not a maybe, it’s well established. For some patients, that’s the goal. For others, it’s exactly what we’re trying to avoid.

XERF, on the other hand, delivers gradual heating deeper into the skin, with a much lower risk of fat loss. Think maintenance, not depletion.

So which is better? It depends on your face and your end goal.

• Want subtle fat reduction + lift → HIFU may suit
• Want to preserve volume + improve tightness → XERF is often the smarter long-term play

There’s no one-size-fits-all. Just the right tool, used at the right time.

Guys, so you want to know about Zerf? What is Zerf and how does it differ from HIFU? And why do we think we need both treatments? So, Zerf itself is monopolar RF. Monopolar RF, you can think of it that's slow heating, while HIFU, much like lasers, is deep heating. Lasers are totally different because you have to treat the upper layers of skin, while HIFU itself treats the upper, but also lower areas of skin. But still, it's considered a fast heat device. Now, with fast heat devices, if it's confined to the dermis, that's fine. But if you continue using fast heat devices in your subcutaneous tissue, much like HIFU, it will eventually ablate or reduce subcutaneous fat. That's proven and that's a fact. So, with Zerf, it uses monopolar RF, which means it's slow heating that's delivered deeper into the layers of skin. With this, you would see several publications that show it can reduce facial fat, but the chances of doing that is much, much less compared to HIFU. So, here's my take. For patients who want a reduction of facial fat with possibly some lifting, HIFU may be the better option. For patients who want to maintain the tightness, especially in the subcutaneous layer with minimal collateral damage to the fat itself long-term, they can have repeated treatments of Zerf. It's safe, but also effective, provided patient selection is met.
